Were you on Unemployment and received unemployment benefits from the state? If so, you should get your 1099-G form so you can file your taxes. For most states, you can log in to your state unemployment account and print your 1099-G form. If you can’t find it, please call your state unemployment call center for a duplicated copy.

This is a sample of what a 1099-G looks like. Each state will have a different design but will contain similar information. It will include your name, social security number, address, previous year’s unemployment benefit amount, and any federal or state tax withholding.
